Although Long-term and Post-Acute Care (LTPAC) facilities are not yet eligible for most of the government incentives surrounding EMR implementations, there are still ways to get involved in improving the health of their patients through HIT. How can this be done?

One of the easiest ways is converting MDS files into continuity of care documents, or CCDs. This allows LTPAC facilities to provide information to their local HIE.
